The following should be brought with you to your appointment:

Current Insurance Card(s)

Your insurance cards will be scanned into your medical record.  Payments vary depending on your insurance plan.  Bring insurance co-payment, deductible, or payment in full if you are not covered by insurance.  Payments will be collected when you check out.  

Medicines That You Are Currently Taking

Bring a list of medications with dosage, if possible.  This includes any over the counter medications or herbal supplements.

Medical Records

Prior to your first appointment, verify that your vision records are being mailed or faxed from your previous doctor.  This will help your new optometrist coordinate your care.  If you have not already done so, you can complete a records release form at your initial visit.
